Do I need a referral?
If your insurance plan requires you to get a referral from your primary care physician prior to receiving care from a specialist, it is your responsibility to get the referral. This must be done prior to your appointment, as most insurance companies will not accept a request for referral after the service has been rendered. If you do not get a referral, your insurance is not required to pay, and you will be responsible for the payment of all charges.
Is a co-payment required?
This is determined by your insurance coverage. If your insurance requires a co-payment, we appreciate your payment at the time of your appointment.
Are your services covered by my insurance?
All medically necessary foot and ankle care is usually covered by insurance. There may be some exceptions as determined by your particular insurance policy. If you have any questions, contact your insurance company or our office for additional assistance.
What if I have do not have insurance?
We ask you to pay for your initial services when they are rendered. Payment plans can be arranged if necessary. In addition, credit cards are accepted for payment.
What do I need to bring for my appointment?
- Your primary care physician's name and address.
- A list of other physicians you currently see.
- A list of your current medications.
- Your insurance card(s) and referral information.
- Your co-payment.
